Chapter Membership and Release Form Use this form if you are an adult who desires to join our HOG chapter. It includes a release to enable you to participate in HOG events.
Chapter Event Release form for Adults Use this form if you are an adult who is not a HOG member and you desire to participate in a HOG event yourself.
Chapter Event Release form for Minors Use this form if you are an adult parent and you desire that your child participates in a HOG event.
Minor's Assumption of Risk Acknowledgement - Use this form if you are a minor and you have parental consent to participate in HOG events.
